Register at the CUSA Office. This is the only time you must register at the office.

You will need to bring a government ID with your photo and date of birth.

  • Accept your online invite
  • Come to the office

  • Complete and submit to the office your waiver via email
The first time you register as a member of CUSA you will need to submit a Youth Information Sheet that must be signed by a legal guardian and yourself.

Players need a valid player card for the team they are playing for. Players are responsible for their own player card.

For the majority of the divisions there is no deadline to register. Players can register up to one day before the final game of the season for their team. The exception is for those teams playing in a division that leads to a CSA competition - the final day to be on a roster is January 31.

Player Card Costs
New Players       $20.00
Returning Players Changing Teams With Old Player Card       $10.00
Returning Players Changing Teams Without Old Player Card       $25.00

A player's card must have the correct club or team name on it to be valid.

Having a player card but not submitting a waiver does not make you eligible to play. You must be registered in the current season before you play.
